Please use a valid email and phone number on intake forms in case we need to contact you about changes to your appointment. Less than 24-hr cancellations or no-shows require full payment which will be applied on next booked appointment or billed directly. Patients may be blocked from future bookings.
Arrive on time your appointment and dress comfy. Preferably no jeans or dresses, but it's ok if you forget or cannot.
We have a waiting room please come into the bunkie at the back of the property, also bring your insurance and credit card on your first appointment.
